New Telehealth Patients:
Please take time to review and complete the Patient Intake Forms before your first appointment.
Send your completed forms, along with a copy of your government-issued ID (e.g. state ID or driver’s license) and a copy of your health insurance card (front and back) to office.dcmhc5@gmail.com at least 24 hours before your first scheduled appointment.
* Your payment is your responsibility. All co-pays are due at the time of service. Please call us to pay over the phone by card or mail a check to Donarski Center for Mental Health Counseling. If you do not know your co-pay and deductible information, please contact your health insurance provider directly.
