If you are a new client, please download and complete the appropriate intake form from the New Client Forms section, then bring it to your first session or email it to our office. 2025 Returning Client Information Update Forms
In the New Year, we need updated information from every client so please download and complete the appropriate form for each person who is a client. Additionally, we need a copy of your insurance card. You may bring it into your appointment or you may take pictures of the front and back of the card and email it to us along with completed forms.
